Transitions Transitions is a volunteer-based program designed to provide non-medical support and practical assistance for those who have a progressive illness. A Transitions Coordinator meets with you and your family to assess your needs, then coordinates your volunteer services and community resource information. Volunteers are specially trained, experienced and caring. They offer support, friendship and respite for patients and their families. They help with transportation, errands, respite for the caregiver, food preparation, friendship and more. The Transitions Coordinator serves as your guide, helping you identify needs, providing emotional support for decision making and pointing clients toward valuable community services. Anyone can refer a person to Transitions by calling (941) 302-3800, (941) 552-5962 or 888-202-9972. The Transitions Coordinator will make an appointment to meet with the prospective client and his/her family. The Coordinator will also contact the client's doctor, after admission, to inform the doctor of participation in the program. Transitions is available to anyone who has been diagnosed with an advanced illness and has a prognosis of one year or less. There are no charges for services provided. The program is fully funded through TideWell Hospice and Palliative Care. Transitions volunteers and staff visit clients in the hospital, at home or at an extended care facility.
